If you have a sport or camp form you need completed and signed, please call our office and speak to a receptionist. We can advise you of the most convenient way to get this done. Most forms require the exam be done within the last 24 months. If your child had a well child exam within the time frame specified by the form, we can usually complete and sign the form without having to see your child again.
Per the American Academy of Pediatrics, the American Medical Association, and U.S. Preventive Task Force Services, the current recommendations for everyone 4 years and older is to have a complete physical exam every twelve months. If your child has not had a complete physical exam (also known as a well child exam) within the required timeframe, we would strongly encourage you to schedule one. These exams are generally covered by most commercial insurance plans. If you have any questions about your coverage for these exams, please contact your personal insurance plan.
Pediatric Associates of Gresham will schedule and perform a sport or camp clearance exam if needed. A nominal charge is due at the time of the visit for these exams. Because of the brief nature of these clearance exams, they are generally not covered by insurance, nor are they meant to take the place of a complete physical.
